Q: What forms of payment are accepted?
A: Winning Wheels accepts many forms of payment, including Medicaid, Medicare, VA contracts, private pay and insurance.

Q: How many residents live at Winning Wheels?
A: Winning Wheels has a licensed bed capacity of 80.

Q: What is the average length of stay??
A: The average length of stay by diagnosis is listed below, and changes annually.
  • Brain Injury = 7 years, 8 months
  • SCI = 4 years, 8 months
  • MS = 8 years, 8 months
  • Other = 17 years, 3 months

Q: What is the average age of Winning Wheels residents?
A: Currently, the average age of a Winning Wheels resident is 43, ranging in age from 19 to 65.

Q: What services does Winning Wheels provide?
A: Winning Wheels provides 24-hour nursing care, physical rehabilitation, occupational rehabilitation, therapeutic recreation, speech therapy, cognitive rehabilitation, day treatment, music therapy, woodshop activities, beauty and barber services, nutrition management, and skilled therapy as appropriate.

Q: What are the admission criteria?
A: Wheelchair dependent or semi-dependent, rehabilitation potential and a high level of motivation.

Q: What is the admission process?
A: The Winning Wheels admission process is outlined below.
  1. Applicant sends in preliminary application, which can be found online at www.winningwheels.com or by calling 815-537-5168, along with medical records, therapy records, notes, etc.
  2. Winning Wheels' Admission Committee reviews the application.
  3. If necessary, the Admission Coordinator will arrange a meeting with the applicant.
  4. The Admission Committee makes the decision as to whether or not the applicant will be appropriate for Winning Wheels and will benefit from the services we provide.
  5. The Admission Coordinator will notify the applicant via mail. If applicant has been approved they are placed on the waiting list.
  6. As beds become available, the Admission Committee chooses an applicant from the waiting list who would be the best fit at that time.
  7. The Admission Coordinator contacts the applicant to arrange for admission to Winning Wheels.

Q: Do you accept patients with ventilators?
A: Not at this time.

Q: How long is the waiting list?
A: The length of time on the waiting list can be anywhere from a few months to a few years, depending on the number of people being added to the waiting list as compared to the number of residents who are being discharged.

Q: Where is Winning Wheels located?
A: Winning Wheels is located on a picturesque 12 wooded acres in the rural Northwestern Illinois community of Prophetstown.

A: Please contact the Illinois Brain Injury Association at

A: Other LTC facilities in the area include

Q: Does Winning Wheels have national accreditation? Yes! What is CARF?
A: Winning Wheels has been awarded CARF accreditation. CARF, the National Accreditation for Rehabilitation Facilities, is an independent, not-for-profit, international accrediting body whose mission is to promote the quality, value, and optimal outcomes for services through a consultative accreditation process that centers on enhancing the lives of the persons served. Founded in 1966 as the Commission on Accreditation of Rehabilitation Facilities, and now known as CARF, the accrediting body establishes consumer-focused standards to help organizations measure and improve the quality of their programs and services.

Q: What is Winning Wheels' direct care staff ratio per patient?
A: A licensed nurse is always on duty on each of our wings (a 1:40 ratio) and at least 4 CNAs for 40 patients during day/evening shifts. These numbers are our absolute minimum and we typically have more nursing staff on duty as well as ancillary staff who are available to assist residents.

A: The list of Winning Wheels' discharge outcomes from 2005 are listed below.
  • Independent Living = 62.5%
  • Other = 25%
  • Nursing Home = 12.5%
